- Worked with a Local Authority and Primary Care Trust to accelerate the
co-ordination and implementation of the Single Assessment Process for older
people. Involved working across primary care, community health/social care
services and the hospital/acute sector. As an adjunct to this work,
developed a joint electronic health/social care assessment record to support
SAP.
- Assisted several Primary Care Trusts and Local Authorities with implementation
of various parts of the National Service Framework for Older People, especially
developments in relation to intermediate care, age discrimination and
information needs.
- Devised new multi-agency joint commissioning arrangements for mental
health services in a large London Borough. Involved analysing existing
systems and processes, undertaking an options appraisal exercise and
preparation for implementation, using the flexibilities under s.31of the
Health Act, 1999. Budget for new joint service was c£25million p.a.
- Developed and wrote a range of policies and eligibility criteria for a Health
Authority (popn. 460,000, revenue budget c£265m) concerning NHS continuing
health care, working to national Department of Health deadlines. Subsequent work
included reviewing the implementation and effectiveness of these policies for
five other Health Authorities in the South of England.
- Improved the quality of residential and day care services provided by a
London-based charity by evaluating its activities regarding care planning,
monitoring, reviews and staff supervision. Designed a set of clear written
standards by which performance could be measured and monitored.
